Brazil probe shows severe icing contributed to 2024 plane crash that killed 62

Brazilian air force investigators have released their findings regarding the tragic Voepass airplane accident that occurred on August 9, 2024, resulting in the death of 62 individuals. The ATR 72-500 aircraft crashed in the residential area of Vinhedo, while en route from Cascavel to São Paulo’s international airport.

The investigation revealed that the accident was precipitated by severe icing conditions, mechanical system failures, and distractions faced by the flight crew. At the time of cruising at 17,000 feet, the plane encountered significant atmospheric icing that caused a loss of lift, leading the aircraft to enter an unrecoverable "flat spin."

Flight recorders indicated that the crew acknowledged the heavy accumulation of ice on the airframe. However, issues with the aircraft’s de-icing system, which were recurrent and known prior to takeoff, were not adequately addressed. The system was frequently toggled on and off during crucial flight phases, impacting performance.

Investigators indicated a concerning “culture of normalizing deviations” within Voepass, where these mechanical issues were tolerated, contributing to the disaster. During critical icing warnings, the crew also became distracted by routine communications and administrative responsibilities, delaying vital corrective actions.

The findings were shared with the victims’ families before being disclosed publicly in a press conference held in Brasília.
